YoloCares CalAIM and Supportive Care Outreach Coordinator will play a crucial role in supporting the implementation of the CalAIM and Supportive Care initiatives by coordinating outreach efforts to engage eligible individuals and communities. This role involves extensive community engagement, partnership building, and collaboration with various stakeholders to promote awareness and access to CalAIM and Palliative Care services.
Essential Duties And Responsibilities
- Advocates for patient health needs.
- Supports and promotes the mission of the Agency.
- Works closely with the palliative care team, hospice team, and volunteer coordinators to plan and deliver appropriate support, resources, and counseling services to palliative care patients, ECM and Community Support clients.
- Coordinating referrals for community services, assessing client needs and creating individual plan of care, work in close collaboration and communication with hospice and palliative care case managers and act as a liaison with supervision leaders to assess patient needs.
- Ability to explain health forms, diagnosis, and processes clearly with cultural responsiveness to the patient and their family.
- Documents all assessments and interactions with patients in a timely manner.
- Collaborate with an interdisciplinary team to determine and suggest a higher level of care for the client.
- Provides consultation to the IDT members, healthcare staff and collaborates with community referral agencies/persons, observing confidentiality regulation and policies.
- Maintain appropriate client information adhering to agency standards.
- Maintain strict confidentiality in accordance with agency policies.
- Develop and implement outreach strategies to identify and engage eligible individuals for CalAIM services.
- Conduct community presentations, workshops, and informational sessions to raise awareness about CalAIM.
- Utilize various communication channels (social media, print, radio, etc.) to disseminate information about CalAIM and Palliative Care.
- Establish and maintain relationships with community-based organizations, healthcare providers, local government agencies, and other stakeholders.
- Collaborate with partners to coordinate outreach activities and leverage resources.
- Assist individuals in understanding CalAIM and Palliative Care services and eligibility criteria.
- Provide one-on-one support to individuals and families to navigate the enrollment process for CalAIM services.
- Maintain accurate records of outreach activities, contacts, and outcomes.
- Work closely with the ECM and Community Support Manager and/or Director of Palliative Care to develop and execute outreach plans aligned with CalAIM and Palliative Care goals.
- Monitor and evaluate the effectiveness of outreach strategies and make recommendations for improvements.
- Participate in training and professional development opportunities related to CalAIM, Palliative Care and outreach best practices.
- Collect and maintain data on outreach activities, participant demographics, and service utilization.
- Prepare regular reports on outreach efforts and outcomes for internal and external stakeholders.
- Maintain client confidentiality and high ethical and professional standards.
- Report to work promptly at assigned hours and submit accurate time sheets/reports.
- Inform the Administration of any illegal or harmful (or potentially so) situations regarding the operation of programs and clients.
- Follow State and Federal laws regarding confidential information and child abuse reporting.
- Maintains a positive, life-giving environment within team dynamics and during community outreach engagements.
- Attends training and all necessary meetings.
- Other duties and responsibilities as assigned.
- Has lived experience personal or professional with the homeless popualtion.
- Has or willing to obtain Community Healthcare Worker Certification within 18 months of employment.
- High school diploma or equivalent; Additional education or background in Public Health, Social Work, Community Development, or a related field is preferred.
- Experience in community outreach, public health, social services, or other related fields, preferably with a focus on engaging diverse populations.
- Sufficient understanding of the CalAIM and Palliative Care initiative, its goals, and the services offered.
- Familiarity with social determinants of health and community resources available to target populations.
- Strong communication and interpersonal skills for effective community engagement and partnership building.
- Ability to develop outreach strategies and materials that resonate with diverse audiences and accurately reflect organization’s objectives and services offered.
- Proficiency in data collection and analysis to assess outreach effectiveness and inform strategies.
- Ability to make informed decisions regarding outreach methods and engagement tactics based on community needs and reception.
- Awareness of and sensitivity to the cultural, linguistic, and socioeconomic diversity of the communities served.
- Credentials or relevant training in community engagement strategies, cultural competency, specific services offered under Palliative Care, CalAIM, and/or DHCS best practices.
- Willingness to build and maintain strong partnerships with community organizations, healthcare providers, and/or stakeholders to enhance outreach efforts.
- Effective verbal and written communication skills.
- Able to hear, speak, read, write, and/or type well enough to perform required job duties.
- Able to use hands to finger, handle, or feel.
- Able to occasionally walk and reach with hands and arms.
- Able to lift and carry up to 15 pounds.
- Specific vision abilities required by this job include close vision and ability to adjust focus.
- If driving on YoloCares business, must have a valid California driver’s license and state required automobile insurance.
