Job Summary: The Physical Therapist (PT) will evaluate, plan, treat, and provide direct patient care services using standard physical therapy practices and procedures. The Physical Therapist will also be responsible for documenting patient treatments, changes in condition and other pertinent information as required.

Duties/Responsibilities:

  • Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
  • Assure all treatment is delivered in accordance with an established plan of care
  • Provide clinical support and instruct patients, families, and caregivers
  • Measures & records patient’s motor function, strength, and muscle performance
  • Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
  • Completes documentation in accordance with company policy and state/federal regulations


Skills:

  • SNF/Long term care experience preferred
  • Current knowledge of treatment practices
  • Knowledge of Net Health Electronic Documentation System preferred
  • Ability to manage patients with different types of personalities


Required licensure:

  • Current License as a Physical Therapist in the State of Wyoming. Must be in good standing.
