Social Services Designee

About the Role:

As a Social Service Designee, you will play a vital role in ensuring the emotional, social, and psychological well-being of our residents. You will work closely with residents, families, and interdisciplinary teams to provide support, coordinate services, and enhance quality of life in our facility.

Responsibilities:

• Assist in assessing residents' psychosocial needs and develop individualized care plans
• Provide emotional support and counseling to residents and families
• Coordinate services such as discharge planning, community resources, and advocacy
• Maintain accurate documentation in compliance with state and federal regulations
• Collaborate with nursing and therapy teams to enhance resident care
• Assist with admissions, resident rights education, and grievance resolution
  
  
  

Qualifications:

• Experience in social services, preferably in a healthcare or long-term care setting
• Knowledge of state and federal regulations regarding resident rights and social services
• Strong communication, organization, and problem-solving skills
• Ability to build relationships with residents, families, and staff
