RN Case Manager

We are seeking a compassionate and experienced RN Case Manager to lead and oversee the operations of our Indiana Home Health Division. This is an exciting opportunity for a licensed RN with strong leadership skills and a passion for delivering high-quality, patient-centered care.

Key Responsibilities:

• Lead and manage the daily operations of the home health division, including staffing, budgeting, and patient care oversight
• Ensure full compliance with all state and federal home health regulations
• Develop and implement clinical policies and procedures to uphold high standards of care
• Collaborate with physicians, social workers, and other healthcare professionals to coordinate care plans
• Oversee patient admissions, discharges, and monitor overall well-being
• Manage billing, budgeting, and financial reporting functions
  
  
  

What We’re Looking For:

• Active Indiana Registered Nurse License or compact licensure
• Prior experience in home health care or a related field is preferred
• Proven leadership and staff management experience
• Strong knowledge of medication administration, long-term care, and regulatory compliance
• Excellent communication and organizational skills
• Familiarity with patients’ rights and care coordination

Schedule:

• Monday to Friday (some weekends possible)
