Description
Zumbro Valley Health Center is seeking a flexible, team-oriented, and highly motivated individual to join the Care Coordination team. As a Youth Care Coordinator, you will be responsible for coordinating integrated medical, behavioral, and substance use care for clients.

Since 1966, Zumbro Valley Health Center has been serving our community as a non-profit organization. We serve the twelve county region of Southeast MN and have over 240 employees. We are one of the few providers in the state, to provide such an integrative and comprehensive care to all children, adults, and families. In order to provide an integrative approach we offer a wide variety of services including Outpatient Psychotherapy and Substance Use, Inpatient Residential Treatment, Withdrawal Management and Detoxification, Housing, Skills – Based Training and Targeted Case Management for adults and children, and more.
Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.

Responsibilities
- Coordinate multidisciplinary treatment plan meetings with client, family, and external providers.
- Coordinate communication between client, primary care, behavioral health and other service providers.
- Facilitate, connect, and monitor internal and external referrals.
- Conduct basic health screenings; provide education and referrals for preventative and other wellness service.
- Coordinate discharge/transition planning for clients admitted to the hospital or emergency room, assist with scheduling follow-up within state guidelines, complete required documentation.
- Document and monitor progress in achieving client's goals.
Requirements & Qualifications
- Associate or bachelor’s degree in social work, psychology, nursing or related field required.
- 1 year (2,000 hours) of experience working in the mental health or behavioral science field.
- Must qualify as a Mental Health Practitioner in the state of Minnesota.
- Valid Driver's License and Proof of Auto Insurance.
