Brief Description

Summary of Position: The Care Coordinator provides medical case management, supports the Health Team, helps participants access health services, and assists participants in adopting healthy behaviors.

Specific Duties

  • Provide medical case management:
  • Collaborate with local health care and social service providers
  • Complete professional referrals as indicated by the needs of participants
  • Communicate participant treatment plans to others involved in the participant’s care
  • Assist participants in obtaining and maintaining health insurance
  • Prepare forms (e.g., insurance applications, transportation assistance)
  • Function as a patient navigator for participants for medical services; transport and accompany participants as needed to medical appointments
  • Assist participants with remote access and use of their electronic medical records at other facilities (e.g., EPIC)
  • In consultation with the health providers, refer participants to appropriate medical services as needed
  • Document participant encounters (phone, verbal, email) in the Electronic Health Record (EHR) and the Homeless Management Information System (HMIS)
  • Complete entries in the Homeless Management Information System (HMIS) for all members of the health team

Provide administrative support

  • Coordinate TB screening, flu vaccination and COVID testing for participants
  • Prepare and submit reports (using HMIS, the EMR, and other databases)
  • Maintain all logs and required checks of medical equipment
  • Support drop-in physician clinics
  • Maintain clinic supplies (e.g., inventory, ordering, purchasing)
  • Arrange and maintain examining room instruments, supplies, and equipment
  • Support infection control
  • Maintain all logs and required checks (e.g., refrigerator temperatures, eyewash stations, equipment checks)

Physical Demands & Work Environment: This position requires standing, walking, bending, kneeling, and stooping. This role requires regular travel between various locations.

Requirements

Minimum qualifications:

  • Bachelor's degree or equivalent experience providing care coordinator or medical case management in a healthcare setting
  • Valid driver’s license
  • Effective problem solving, organization and prioritization of tasks
  • Strong verbal and written communication skills
  • Computer competency with Microsoft Office suite and data entry
  • Ability to work independently and be self-directed and flexible

The Following Qualifications Are Highly Desirable

  • Special interest in Medical Care or previous medical training
  • Experience working with persons with housing insecurity, mental illness, and/or have substance use disorders
  • Experience with implementing evidenced-based practices including trauma-informed care and harm reduction
  • Cultural competency in working with a diverse population including those experiencing housing insecurity and/or with behavioral health conditions and a strong commitment to valuing diversity and contributing to an inclusive environment for staff and participants

Other Requirements

  • Must submit to a criminal background check
  • Must submit to or provide evidence of a recent test for tuberculosis
