Multiple Case Manger positions availableResponsibilities:Foster a structured and supportive environment.Establish rapport with consumers.Assist with daily living skills such as cooking, budgeting and personal care. Assist with medication management.Ensure satisfactory functioning of the facility.Provide supportive counseling as needed.Provide encouragement and giving positive feedback.Transport clients and run program errands as needed.Promote a culture of inclusion and belonging.
Requirements: High School diploma or GED required and at least 1 yr of experience working with adults living with a mental illness. Valid NYS Driver License. Reliable transportation. Excellent verbal and written communication skills. Computer skills. Ability to work independently and as a team.
