Agency Requirements
- Valid NYS drivers license
- Use of personal vehicle during work hours
- Clearance of the NYS Justice Center Requirements: Staff Exclusion List (SEL), NYS State Central Registry for Child Abuse and Maltreatment (SCR), Criminal Background Check (CBC)
- Demonstrates knowledge of the program, missions, and services of PCS.
- Complies with all policies and procedures of PCS (Collective Bargaining Agreement, and corporate Compliance Policies) and all governmental and legal requirements.
- Attends and participates in appropriate agency staff trainings and meetings.
- Advocates for cultural diversity/sensitivity as appropriate.
- Reports all suspected incidences of child abuse/neglect to NYS Mandated Reporter Hotline
- Provide direct social work contacts to 15 Non-Medicaid (NMD) youth ages 5-18 and 10-15 Health
- Maintain consistent and on-going contact with youth, families, community providers, schools and any
- Coordinate on an ongoing basis with providers and conduct team meetings biannually to develop a

- Assure youth has access to the Agencys on call services for assistance after hours.
- Provide on call services after hours as scheduled
- Prepare professionally accurate and updated case documentation
- Meet with Program Supervisor on a regular, planned basis
- Participate in mandatory and optional trainings, including but not limited:
- CANS-NY Certification (annual recertification)
- NYS Mandated Reporter training (annually)
- Netsmart CareManager Training
- Engagement and Outreach
- Safety in the Community
- Trauma Informed Care
- Person Centered Planning
- Cultural Competency/Awareness
- LGBTQ Issues
- Meeting Facilitation
- Monthly Group Supervisions
- Advocate for cultural diversity, as appropriate
- Perform other duties as assigned by Administration
NON-MEDICAID JOB RESPONSIBILITIES
- Conduct CANS-NY Assessments for the NMD youth to determine level of care and needs
- Enter youths demographic and assessment information into CAIRS for state level tracking
- Verify Medicaid status on a monthly basis or as frequently as needed
- Provide developmentally appropriate support and linkages for employment, education, housing and other resources to help maintain youths health and develop skills for adulthood
- Engage YA in their goal planning process
