Brief Description

Job Objective:

Under the direction of the Administrator, to provide accurate maintenance of operational and clinical data relating to patient medical records. Maintain a cost effective inventory system of departmental medical supplies.

Essential Duties and Responsibilities:

  • Records Coordination
  • Creation and tracking of department patient master charts.
  • Accurate filing in patient medical record of all legal patient documentation including but not limited to consents, clinical notes, physician orders, lab reports in a chronological order by discipline
  • Coordination of medical records requests and/or legal subpoenas.
  • Communicates regularly with practitioners regarding incomplete charts due to unsigned orders.
  • Runs incomplete chart reports as defined by procedure.
  • Provides access as needed to the patient master chart to field staff.
  • Reviews each discharged patient’s chart for incomplete or missing items and identifies the responsible practitioner or discipline for each of these items.
  • Replenish forms, packets and field charts for clinical staff as needed.
  • Responsible for maintenance of department medical records at off site storage facility.
  • Primary liaison with AVMC Medical Records Department.
  • Inventory Systems
  • Maintains adequate and organized medical supply inventory for department staff.
  • Responsible for requisition submission, stocking and monitoring usage of medical supplies for quality control and cost effectiveness.
  • Accurate data entry of staff supply requisitions.
  • Monitor usage and maintain stock of department specific and/or Medical Center forms.
  • Research and reporting to managers of special order or non-stock medical supplies.
  • Primary liaison with AVMC Material Services.
  • Operations
  • Answers telephones consistent with Service Excellence standards, addresses and/or routes calls appropriately and in a timely manner.
  • Interacts with department staff, Medical Center staff, medical staff, other coordinators, and managers to ensure prompt and accurate communication as needed to achieve the department’s goals.
  • Accurate data entry of employee timesheets.
  • Coordination of department volunteers.
  • Responsible for mailing and/or replenishment of marketing materials.
  • Consistently meet established departmental and Medical Center processing deadlines.
  • Participate on Project Teams related to internal process improvement and Medical Center policies as needed.
  • Safety Officer
  • Performs safety officer duties as defined by Safety Committee.
  • Prepare and presents safety report at department meetings and educates staff as needed.
  • Orients new employees to offsite safety practices.

Non-Essential Duties:

  • Work related errands involving local travel, including, but not limited to, Medical Center, physician offices, or storage facilities.
  • Provide backup coverage to Home Care Office Coordinator and PAS III as needed.
  • Participate and assist with special projects and assignments as requested.
  • Participates in orientation and education of other personnel.
  • Participates in Quality Improvement (QAPI) process and PI Committees
  • Other duties as assigned.

Knowledge, Skills and Abilities:

Knowledge

  • Knowledge of office practices and procedures.
  • Working knowledge of medical terminology.
  • Knowledge of alpha and numeric filing systems.
  • Working knowledge of Microsoft word processing and spreadsheet applications and/or comparable software

Skills

  • Establish and maintain accurate and systemic records.
  • Typing of at least 30wpm.
  • Strong organizational skills
  • Strong oral and written communication skills.

    • Detail oriented.
    • Ability to utilize basic business data processing principles.
    • Ability to apply processes and policies to practical use.

    Education and Experience:

    Education

    • Minimum High School Graduate or equivalent.

    Experience

    • Must have 1-2 years of office experience, medical office setting preferred but not required.

    Required Licensure and/or Certifications:

    • Maintain a valid California driver’s license and evidence of current auto insurance.

    Key Physical Requirements and Working Conditions:

    • Work usually performed in a normal office environment; however, may require trips outside in inclement weather to the Medical Center, physician offices or to offsite facilities to retrieve and/or deliver reports, Medical Center records or health information.
    • Must meet job physical standards: vision, hearing, and manual dexterity, ability to walk and meet lifting requirements.
    • Must be able to tolerate repetitive hand and arm movements
    • Able to lift and/or carry up to 10 pounds
