Position: Director of Care Management

Job Type: Exempt - Full-time

Schedule: Monday to Friday

Reports to: VP of Clinical Operations

This role is 100% remote with no face-to-face contact with patients. We have a “remote first” environment and are proud to have team members located nationally.

Summary

The Director of Care Management at Lumata Health leads the virtual eye care coaching team, providing strategic and operational oversight. Reporting to the VP of Clinical Operations, the role focuses on building frontline leadership, developing talent, and achieving measurable outcomes through performance metrics. It is key to aligning people, processes, and strategy to ensure high-quality patient care and sustainable growth. The ideal candidate is a hands-on, inspirational leader with experience in healthcare operations, particularly in fast-paced, remote environments. A background in ophthalmology and a track record in workforce planning and cross-functional collaboration are essential.

Essential Functions

Team Leadership & Culture

  • Lead and cultivate a high-performing frontline leadership team, promoting professional development, engagement, and a strong culture of accountability across the virtual care coaching staff.
  • Champion a collaborative, inclusive, and transparent work environment through effective communication and alignment of goals across teams.

Performance & Operational Excellence

  • Establish and manage data-driven KPIs and performance scorecards to drive team productivity, operational efficiency, and quality care outcomes.
  • Partner with the Quality & Training team to translate insights into actionable improvements, ensuring effective implementation of quality initiatives.
  • Oversee hiring, onboarding, and continuous professional development to maintain a capable and engaged coaching workforce.

Workforce Management & Capacity Planning

  • Direct workforce planning to align team capacity with patient volume, ensuring optimal resource allocation and operational efficiency.
  • Collaborate with Patient Enrollment to synchronize staffing with onboarding needs, facilitating smooth and scalable integration into Lumata Health programs.

Strategic Program Development

  • Lead the implementation of new service lines and care programs in alignment with the evolving care coaching model and organizational goals.
  • Provide strategic input on growth initiatives, ensuring teams and systems are prepared to support new offerings.

Compliance & Clinical Support

  • Ensure adherence to all regulatory, contractual, and clinical certification standards in coordination with the company’s Quality Compliance Plan.
  • Forecast staffing needs and manage clinical headcount in alignment with program growth and budgetary targets.

Cross-Functional Leadership

  • Represent the Care Management function at the senior leadership level, advocating for resource needs, operational priorities, and cross-departmental alignment to support strategic objectives.

Requirements

Required:

  • Bachelor’s degree in a healthcare-related field or equivalent experience.
  • 10+ years of expeirence in healthcare operations leadership or senior management.
  • Proven experience leading frontline healthcare teams, implementing operational workflows, and driving performance using data-driven metrics.
  • Demonstrated ability to hire, mentor, and develop remote teams, fostering a high-performing and client-focused culture.
  • Strong leadership, communication, collaboration, and change management skills, with a track record of independent, results-driven work.
  • Proficient in leveraging data for decision-making, managing compliance, and ensuring documentation standards are met.

Preferred

  • Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW) credential.
  • 5–10 years of experience as a social worker, with a demonstrated ability to manage teams and lead operational initiatives.
  • Proven experience scaling teams in a fast-paced, high-growth healthcare services or startup environment.
  • Expertise in developing and directing comprehensive care plans that align with both client needs and organizational goals.
  • Strong background in coordinating and integrating community resources to improve service delivery and efficiency.
  • Experience in eye care or virtual care delivery models is highly desirable.
  • Skilled in workforce management and resource scheduling to support operational scalability.
  • Familiarity with launching and operationalizing new healthcare programs or service lines.

Physical Requirements

  • Must have a home office or HIPAA-compliant workspace
  • Prolonged periods of sitting and computer use
  • Must be able to hear, speak, and conduct virtual meetings

Work Environment

In order to perform the essential functions of this job, you must have a dedicated remote workspace, and a wireless connection with a minimum of 50 Mbps of download speed and 10 Mbps of upload speed of dedicated internet bandwidth for each person working from home; the workspace is private and has low to no noise level as you will be on phone calls with patients. It is very important that your workspace is HIPAA compliant; your company issued laptop/pc must maintain visibility that only you can access as you'll be handling patient data. You are also expected to have your camera on during virtual meetings, therefore, you must be in a designated workspace/room for privacy.
