We are hiring compassionate & dedicated LICENSED PRACTICAL NURSES (LPN'S) to provide medical support to our clients.
Duties and Responsibilities:
We are hiring compassionate & dedicated LICENSED PRACTICAL NURSES (LPN'S) to provide medical support to our clients.
Duties and Responsibilities:
- Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Provide direct nursing care to patients in their homes under the supervision of an RN or physician.
- Administer prescribed medications, treatments, and therapies accurately.
- Monitor patient health, vital signs, and symptoms, and report changes promptly.
- Educate patients and family members on proper home care techniques and health maintenance.
- Maintain accurate, detailed, and timely nursing documentation.
- Perform wound care, catheter care, and tracheostomy care as needed.
- Support patient emotional well-being and provide compassionate end-of-life care when necessary.
- Ensure compliance with all healthcare regulations, agency policies, and individualized care plans.
- Assist with mobility and transfers, ensuring patient safety and fall prevention.
- Conduct patient assessments during each visit and update care plans as necessary.
- Participate in training sessions and professional development to maintain nursing licensure and skills.
- Valid LPN license in NY (REQUIRED)
