Essential Functions
- Provide direct nursing services for individuals served by the day habilitation program at the agency. Propose recommendations for health and safety policies and procedures.
- Under the supervision of the Healthcare Supervisor, assist staff in designing and implementing health/safety training for individuals to maximize highest level of independence through ongoing education and staying current in medical field.
- Attend DHSP and Case Conferences, Intake meetings, Interdisciplinary team meetings and Discharge Planning meetings as necessary to ensure comprehensive services for individuals receiving services.
- Assist Healthcare Supervisor with initial/annual nursing status assessments for day habilitation participants. Write and implement Nursing Care Plans and medical protocols as needed. Maintain monthly progress notes and update seizure plans annually. Ensure compliance of medical records as required by agency's policies, DMA, DDS, DPH, and CARF guidelines.
- Perform all duties in a manner which demonstrates an awareness and respect of each individual's legal, civic and human rights. Maintain and enforce the confidentiality of any information relating to individuals served. Act in an unbiased, neutral and fact base manner at all times.
- Provide skilled nursing care and medication administration as prescribed by the health care provider
- Perform job duties in close cooperation with other departments fostering a work environment of mutual cooperation and support. Work collaboratively with team members, actively coordinating goal development, meetings, and maintenance of program areas/spaces. Work as a positive team member, supporting other staff during interventions, activities, and other duties as assigned by supervisor.
- Order medical supplies and medication as necessary.
- Complete all mandatory trainings to ensure skill maintenance and development. This includes PAC, CPR and First Aid, trainings mandated by Waystone, as well as other department and state agency required trainings and certifications.
- Provide transportation as requested by supervisor
- Assist in maintaining a clean and safe environment by reporting issues to the appropriate personnel. Maintain professional and organization nursing space.
Requirements:
- Current Massachusetts LPN license
- Experience working with people with intellectual and developmental disabilities
- Clinical, organizational and communication skills
- Valid driver’s license in good standing with minimum of 1 year driving experience.
- Regular and predictable attendance
