Description

Duties and Responsibilities:

  • Responsible for documenting treatment plans, progress notes, treatment plan updates, case consultations, telephone calls and discharge summaries in the patient records.
  • Documents summations of the clinical process through which patients address treatment goals, as stated on their treatment plans via an interactive process with peers and other patients.
  • Family case management – Collects information regarding the patient and family system, assesses family dynamics, recommends the family education program and possible ongoing family therapy.
  • Responsible for group facilitation.
  • Continuing care planning and case management – Understands unresolved and deferred goals and objectives in the continuing care/aftercare plan, and supports the continuing care specialist with releases, appointments, etc.
  • Expected to work within Caron’s expectations of policies and procedures that support ethical patient care.
  • Responsible for completing documentation necessary to support utilization review.

  • Must be able to work extended hours.
  • Must be able to deliver presentations that may be both multiple times and for lengthy time periods.
  • Must be able to lift up to 20 lbs.
  • Must be able to ascend and descend stairs and move throughout Caron properties.

Education / Experience / Qualifications

  • Master’s degree in Human Services with 6 months clinical experience or bachelor’s degree with one-year clinical experience required.
  • Training in a related health field preferred.
  • Experience in the area of facilitating group therapy. Experience in addressing developmental issues for a wide range of age groups and within their respective fields.
  • Must meet required clearances.
  • If in recovery, 2 years of uninterrupted sobriety preferred.

  • Must possess the willingness and capacity for growth, both personally and professionally.
  • Must have the ability to be objective, empathetic while addressing the individual needs of the respective patients, referrals and family members.
  • Acts with consideration for the individual's cultural beliefs, interests, wishes, needs, and values. Case managers are responsible for helping clients and their support systems evaluate and understand care options, determine what is best to meet their needs, and institute action to achieve their goals and meet their interests and expectations.
  • Must be computer literate and able to work with most Microsoft applications along with network components.
  • Demonstrates fluid communication skills and the ability to document insights and observations in a clinical record.
  • Must possess a high degree of professionalism and be able to keep confidential information and treat sensitive clinical information ethically.
  • Must be able to keep all clinical work within the guidelines of ethical standards throughout their work.
  • Must possess ability to coordinate care that is safe, timely, effective, efficient, equitable, and patient-centered.
  • Must be thoroughly familiar and be able to demonstrate knowledge of the principles and programs of the various recovery fellowships.
  • Demonstrated ability to communicate verbally and in writing.

Competencies / Measurements

  • Ability to explain assessment results, identify and rank problems based on individual needs, formulate agreed upon immediate and long or short-term goals and identify treatment methods and resources to be used in developing the overall treatment plan for each patient. -- Observation, treatment plan, problems lists, chart reviews.
  • Must possess the knowledge and skills in the area of assessment and possess the ability to diagnose, and evaluate psychological, social and physiological signs and symptoms of alcohol and drug abuse and other coexisting conditions in order to formulate a diagnosis. – Observations, chart review bio review and performance evaluation.
  • Ability to identify strengths, weaknesses and goals for the development of a treatment plan with individual patient involvement. – Observation, treatment plans, problems list documentation.
  • Maintaining appropriate referral relationships through feedback, satisfaction surveys.
  • Ability to professionally collaborate and interact with families and referents to support integration of collateral information, provide thorough progress updates, and create space for support as needed. Observation, performance appraisal, patient and family satisfaction.
  • Ability to coordinate all services for client care. Able to recognize the elements of the client crisis, implement an immediate course of action appropriate to the crisis, and utilize all Caron resources to resolve and treat the patient group and family in a timely manner. – Observation, patient satisfaction, referral satisfaction, performance appraisal.
  • Able to identify needs and problem areas that are beyond the scope of care by Caron. Able to explain this rationale to referral, patient and family. Able to complete referral to appropriate level of care to meet patient needs.
  • Able to coordinate all aspects of continuing care including but not limited to, establishing first appointments, completion of appropriate release forms, securing funding, arranging transportation, communication with all parties involved in patient care and identifying and coordinating continuing care for issues related to addiction such as mental health and medical care.
  • Able to prepare reports and maintain electronic records by integrating all patient and family information. Must be able to complete all documentation, including continuing care and compliance calls according to Caron format and within the specified time frames.
