Program And Role Summary
- Wayfinder’s Post Adoption Service s (PAS) program offers a variety of support groups, classes and social events for families. Adoption is a lifelong process. Not all needs present themselves immediately but rather arise months – even years – down the road. We are committed to supporting adoptive families today and well into the future.
Primary Responsibilities
- Establish and maintain trusting relationships with adoptive families, ensuring continued post-adoption support.
- Conduct home visits when necessary, virtual check-ins, and in-person meetings as needed.
- Conduct psychosocial assessments to evaluate family strengths, needs, and risk factors.
- Develop and implement individualized family support plans in collaboration with parents and caregivers
- Offer immediate crisis intervention and de-escalation strategies for families facing acute challenges.
- Maintain professional integrity and ethics.
- Other duties as outlined in the position description.
Education and Experience:
- A master’s degree in social work, Psychology, Sociology and/or Communication is required.
- Prior experience in a case management position or equivalent experience
- Knowledge of crisis intervention and the unique needs of kinship care and adoptive families
- Strong knowledge of and ability to deliver trauma-informed practice
- Ability to work in MS Word, Outlook and Excel at intermediate level
- Proficient in using virtual video technologies and have stable internet service with privacy to meet with clients virtually
- Work in adoptions preferred.
