What We Are Looking For

Aspire365 is actively seeking a Case Management Supervisor to join our dynamic multidisciplinary team, dedicated to providing compassionate care to individuals and families. The salary range for this position is $65,000 to $75,000 annually, with significant potential for higher earnings based on performance and growth opportunities. This is a full-time in office position.

Case Manager Responsibilities

  • This position is an in-office position. Aspire365 offices are located in Sandy, Utah
  • Advise other case managers as they care for clients
  • Recommend ideas and strategies to coordinate care for a diverse set of clientele
  • Hold weekly meetings to meet with Case Managers in your team to coordinate the best quality of care
  • Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; monitoring staff performance.
  • Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
  • Admits new clients by reviewing records and applications, and conducting orientations and assessments.
  • Monthly scheduled appointments with clients according to appointment schedule.
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources.
  • Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services, obtaining additional resources; intervening in crises; providing personal support.
  • Maintains clients’ records by reviewing case notes, logging events and progress.
  • Communicates clients’ progress by conducting monthly coordination meetings and evaluations with client’s clinician team.
  • Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; providing resources.

Education, Experience, And Licensing Requirements

  • High School Diploma or GED
  • bachelors degree preferred
  • 3+ years customer service experience, preferably in human services, possess demonstrated commitment to customer service
  • At least one year in a Supervisory or Lead role
  • Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred
  • Must be over 21 years old
