Description

The Behavioral Health Navigator (BHN) is committed to removing barriers to care by identifying critical resources for patients and helping them navigate through behavioral health and substance use disorder services in the Central Oregon region while promoting patient health. The Behavioral Health Navigator will support the behavioral health department by supporting administrative tasks and assisting in quality improvement projects.

Responsibilities

  • This position operates as an extension to the primary care behavioral health team (PCP; MA; RN; BHC, MH Clinician and CADC) and coordinates care for patients both within and outside of the clinic.
  • Navigator will follow up upon and continue outreach support for all external referrals placed for behavioral health care. This includes but is not limited to: specialty mental health; psychological testing; family, individual, martial counseling, SUD treatment options; and psychiatric and psychological care.
  • Schedule and coordinate Behavioral Health and SUD/MAT trainings for staff in partnership with internal resources. Partner with HR to document/track training.
  • Coordinate and attend community partnership meetings/gatherings. Meet with community partners to assure solid and mutually beneficial professional relationships.
  • Attend to Behavioral Health Quality Improvement initiatives under direction of BH Director.

Patient Support

  • This position operates as an extension to the primary care behavioral health team (PCP; MA; RN; BHC, MH Clinician and CADC) and coordinates care for patients both within and outside of the clinic.
  • Navigator will follow up upon and continue outreach support for all external referrals placed for behavioral health care. This includes but is not limited to: specialty mental health; psychological testing; family, individual, martial counseling, SUD treatment options; and psychiatric and psychological care.
  • Creates optimal experience for patients and families, participates in care coordination with other healthcare professionals, family, and community partners to facilitate care for patients, and arrange supportive services as needed.
  • Help with design and marketing of BH/SUD groups within Mosaic. Connect with internal partners to ensure strong communication and population reach within Mosaic patient population.
  • Coordinate Behavioral Health (BH) and Substance Use Disorder (SUD) groups, including coordinating location, prepare the space, and assist with set up/take down. Assist with documentation needs and data capturing as needed.
  • Monitor BH and SUD pools within Epic for assigned teams.
  • Monitor internal referrals to BH and SUD services such as MAT, ensuring patients are scheduled appropriately and referral status is updated in Epic. Be a resource for PCP and clinic teams regarding BHC scope, current offerings, and capacity including group referrals.
  • Communicate with patients to provide information and/or schedule for BH and SUD/MAT group and individual appointments.

Training and Community Partnership Coordination

  • Schedule and coordinate Behavioral Health and SUD/MAT trainings for staff in partnership with internal resources. Partner with HR to document/track training.
  • Coordinate and attend community partnership meetings/gatherings. Meet with community partners to assure solid and mutually beneficial professional relationships.
  • Attend relevant community workgroups as a representative of the Behavioral Heath team at Mosaic.
  • Attend networking and outreach opportunities as appropriate.

Administrative and Quality Improvement

  • Attend to Behavioral Health Quality Improvement initiatives under direction of BH leadership. Use EHR and relevant tools to (such as Collective Medical) track, coordinate and ensure compliance with CCO Metrics as assigned:
    • Initiation and Engagement Metric
    • Hospital Follow up Metrics- MH/SUD/Hospitalization metrics
Education, certification and licensure: High school diploma or GED required. Bachelor’s degree in related field preferred. Equivalent work experience and similar successful experience may be substituted for education.

Skills & Knowledge: Answers questions accurately about role definition and behavioral health program services. Has competencies on the PCBH program manual and demonstrates understanding of our model of behavioral health in primary care. Demonstrates a foundational understanding of population-based services. Demonstrates skills to participate in workflows across the system, with competencies in organization, computer skills, documentation, and clear communication to multiple team members. Has a basic knowledge base of EPIC, managing referrals and phone calls in the system. Shows effective time management skills, to achieve productivity with projects of priority. Utilizes effective practice management skills to engage in supporting designated BHCs with specific projects such as referrals, groups, and patient management.
