Position Summary
The Behavioral Health Consultant (BHC) will provide behavioral health services in the Marram Health Cetner clinics, engaging patients using a targeted brief therapy model, addressing the behavioral health components that support successful treatment of medical problems, substance use issues, mental health needs and access to community resources and supportive services. Individual, group and/or family therapy may also be provided as needed.

- Successfully complete the University of Massachusetts BHC training
- Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders
- Conduct screenings as indicated, and develop diagnoses and treatment plan to guide behavioral health interventions
- Assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration
- Evaluate crisis situations and apply or access appropriate interventions
- Participate in regular communication with providers to promote utilization of BHC's and appropriate referrals to therapy or other services
- Provide brief interventions to build patient's coping strategies to manage medical illnesses/conditions
- Provide behavioral health interventions as part of an integrated response to a patient's medical and mental health needs
- Respond to medical provider requests for a behavioral health intervention in the patient exam rooms and provide therapeutic interventions when behavioral health issues arise during a medical clinic visit
- Utlize evidence-based practices effectively
- Particpate in weekly peer supervision with other BHC's and/or therapists, focusing on effective client engagement, interventions, clinical outcomes and self-reflection
- Participate in weekly staffing of Behavioral Health Services patients for all Marram locations and support an integrated health care model in addressing their needs
- Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease
- Communicate clearly, be responsive, and engage positively with other clinical departments in the organization and with our community partners
- Teach patients, families, and staff, prevention, quality care and treatment enhancement techniques
- Provide information and referrals to community resources and assist patients in accessing those servces
- Secure releases of information to referral sources as needed
- Other duties as assigned
- Signs, symptoms and treatment of psychiatric disorders and substance use disorders, across the age spectrum
- Knowledge of the behavioral health implications of medical issues
- Proficient at targeted brief therapy
- Treatment plan formulation and modification as progress develops
- Individual, group, and family therapy skills
- Proficientcy with electronic medical record and excellent clinical documentation skills
- Conveys a positive attitude and collaborative style with clinic staff, treatment team, and community partners
- Time management and organizational skills
- Utilization of a trauma informed care approach to service provision
- Maintains strong clinical and ethical standards of practice
- Working knowledge of evidence-based practices, including mental health and addictions curricula, and provision of services in a primary care setting
- Adaptive and flexible work style; open to learning from various perspectives
- Excellent written and verbal communication skills
- Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions
- Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral health conditions
- Should be comfortable with the pace of primary care, working with an interdiscipliary team, and have strong communication skills
- Good knowledge of psycho-pharmacology
- Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions
- Master's Degree in Clinical Social Work, Clinical Mental Health Counseling, and/or Marriage and Family Therapy is required
- LCSW, LMHC, and/or LMFT is required
- Addictions experience and Licensure is preferred
- At least 3 years of clinical experience with various populations including children, adolescrents, families, addressing both mental health and addictions issues is preferred
- Behavioral Health Service provision in a primary care setting is preferred
