Position Summary:

Attain Behavioral Health is seeking a compassionate and detail-oriented Intake/Discharge Coordinator & Assistant to support the smooth transition of clients entering and exiting our behavioral health programs. This role is vital in ensuring continuity of care, accurate documentation, and a welcoming experience for all clients and their families. The ideal candidate will have a strong administrative background, excellent communication skills, and a deep respect for confidentiality and client-centered care.

Key Responsibilities:

Intake Coordination

  • Serve as the first point of contact for new client referrals and inquiries.
  • Schedule and coordinate initial assessments with clinical and medical staff.
  • Obtain and verify insurance, demographic, and clinical documentation prior to intake.
  • Ensure all intake paperwork is completed accurately and uploaded into the EMR (Ritten).
  • Provide clients and families with program orientation and respond to general questions.
  • Collaborate with business development and front office staff to track admission outcomes.


Discharge Coordination

  • Coordinate discharge planning in collaboration with clinicians and case managers.
  • Notify necessary parties regarding planned or unplanned discharges (AMA, completion, transfer).
  • Ensure discharge summaries and documentation are completed in a timely manner.
  • Schedule follow-up appointments, referrals, or aftercare services as needed.
  • Track readmissions and maintain discharge logs for quality improvement and reporting.


Administrative & Assistant Support

  • Assist the Clinical Director with administrative tasks, calendar management, and documentation follow-ups.
  • Support program audits, chart reviews, and reporting requirements as needed.
  • Maintain organized records in alignment with HIPAA and compliance standards.
  • Help track program utilization, outcomes, and client satisfaction feedback.


Qualifications:

  • High School Diploma or GED required; Associate’s or Bachelor's degree in a health or human services field preferred.
  • Valid CDCA certification from the Ohio Chemical Dependency Professionals Board.
  • 1-2 years of experience in a behavioral health, substance use, or medical office setting preferred.
  • Familiarity with EMR systems (Ritten or similar) and HIPAA regulations.
  • Strong organizational, time management, and communication skills.
  • Ability to multitask, work collaboratively in a fast-paced environment, and show empathy in client interactions.
