We are seeking a qualified Occupational Therapist (OT) with a Colorado license to provide home-based care.
- Availability Monday to Friday, 8am-5pm, with flexible weekends as needed.
- Minimum one year of recent OT experience; home care experience preferred but not required.
- Ability to work independently, manage time effectively, and demonstrate strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with EMR experience preferred.
- Responsibilities include conducting home visits, evaluating patients' functional, cognitive, and motor abilities, assessing home safety, identifying equipment needs, developing and updating personalized care plans, and educating patients and families.
- Accurate documentation using electronic devices is required.
- Compensation ranges from $75 to $85 per visit, with flexible scheduling and part-time or PRN options.
- Must have a Colorado OT license and be willing to relocate to Pueblo, CO before starting.
