We are seeking a skilled and compassionate Case Manager to join our team. This role will take place in Santa Ana. The Case Manager is responsible for providing direct services and supportive contacts to the client and/or their families, and developing resources within the community, which are supportive to the recovery of the client. They also function as an advocate for the client, seeing that appropriate services are provided and assists the client in receiving these services as appropriate. The Case Manager deliver clinical treatment to those suffering from substance abuse and assures continuity of care in appropriate assessment, treatment planning, 1-1, referrals, and group counseling. Counselors are required to keep thorough records on each client in their caseload and document them in our EMR System (KIPU)

- CADC-I or CADC-II through CCAPP
- 1 year of case management experience in mental health, substance abuse, or related field
- Bachelor's degree in Psychology, Sociology, or related field
- Maintain confidentiality of records relating to client's treatment.
- Prepare and maintain all required treatment records and reports. Provide service to clients individually and in group sessions, to assist in living with trauma, teaching life skills, money management, etc.
- Develop Treatment Plan within 7 days of admission.
- Collect information about clients through interviews, observations, and tests. Ongoing evaluation of client's physical or mental condition based on review of client.
- Maintain appropriate boundaries with clients.
- Refer patients, clients, or family members to community resources or specialists as necessary.
- Provide services to family members to assist them in understanding, dealing with, and supporting clients or patients.
- Participate in staff meetings
- Modify treatment activities and approaches as needed to comply with changes in the client's status.
- Develop a discharge/after-care plan with client.
- Document interactions with clients using the KIPU system.
