As a Case Manager in the United Healthcare at Home program provides social work services to members in home setting. The Case Manager provides intervention, assessment, and consultation of social, functional, and behavioral health issues affecting chronically ill and high-risk patients in a variety of environments, primarily the home setting. The Case Manager works in collaboration with a multidisciplinary team to identify and access services to close care gaps and improve quality of life of OAH members. They help to manage health problems and coordinate health care services for the patients in accordance with State and Federal rules and regulations.
This includes (but is not limited to) assessment, development, and implementation of plan of care, and ongoing evaluation of patient status and response to the plan of care. Ongoing care and management of members is conducted in collaboration with other care team members.
***This is a field-based position covering the greater Columbia, MO area. You will be in the field approximately 75% of the time going into members’ homes. You will be working from home when not in the field.***
Primary Responsibilities:
- Make patient assessments in home setting or as per business guidelines identifying appropriate resources for member’s needs
- Develop and review care plans in collaboration with interdisciplinary care team
- Assist in coordinating community resources based on knowledge of member’s available benefits
- Support with members who have complex medical needs and may have communication barriers
- Build relationships and work with a variety of populations within the community
- Ability to work with low - income populations with complex social and medical needs including adults with serious mental illness and emotional disturbances, members with substance use disorders, and members with other complex or multiple chronic conditions

Required Qualifications:
- Master’s Degree in Social Work
- Active, unrestricted Social Work License in the state of Missouri (LMSW or LCSW)
- 2+ years of experience in long-term care, home health, hospice, public health, or assisted living
- 1+ years of clinical and case management experience
- Experience working with MS Word, Excel, and Outlook
- Demonstrated ability to keep all company sensitive documents secure (if applicable)
- Dedicated work area established that is separated from other living areas and provides information privacy
- Willing or ability to work Monday-Friday any of our 8-hour shift schedules during our normal business hours of 8:00am - 5:00pm. It may be necessary, given the business need, to work occasional overtime
- Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Reliable transportation and the ability to travel (up to 75%) within the service delivery area
Preferred Qualifications:
- Experience with electronic charting
- Experience with arranging community resources
- Field-based work experience
- Experience working with complex family systems and dynamics
- Experience in Crisis Intervention
- Experience working in Duals (DSNP) Chronic (CSNP) and/or Medicaid environment
- Knowledge of both Medical and Behavioral Health Diagnosis and terminology
- Knowledge of symptom identification and intervention, associated with Behavioral Health and Substance Use disorders
- Knowledge of APS reporting processes
- Knowledge of resources to address SDOH
- Understanding of health disparities among various membership, based on their race or ethnicity; religion; socioeconomic status; sexual orientation; gender, gender identity; age; mental health; cognitive, sensory, or physical disability; geographic location or other characteristics historically linked to discrimination and exclusion.
- Proven background in managing populations with complex medical or behavioral needs
- Proven excellent written and verbal communication and relationship building skills
- Proven planning and organizational skills
- Proven ability to work independently as well as a member of a team

The salary range for this role is $59,500 to $116,600 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
