ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Responsible for Day Habilitation Service Plan development for all individuals attending the program.
  • Maintain confidential records of all consumers who participate in the program in accordance with LifeStream policy.
  • Review monthly progress notes submitted by Personal Advocates for accuracy, content, etc.
  • Completes monthly progress reports when applicable.
  • Accesses clinical services as needed in support of consumer programs and needs.
  • Ensures consumer input into DHSP development, personal interests, and programmatic needs.
  • Ensure consumer training occurs according to established guidelines and appropriate documentation is maintained.
  • Maintain regular contact with program manager to ensure appropriate implementation and progress on consumer DHSP goals and objectives.
  • Maintain contact with DDS, Residences, and Parents/Guardians as needed.
  • Act as a member of the team offering input into all programmatic services, schedules, systems, etc.
  • Make recommendations regarding program quality, consumer choices and provide general input to program manager as needed.
  • Provide direct services/coverage when required and requested by program manager.
  • Makes recommendations to program manager relevant to staff training needs.
  • Attends and actively participates in all DHSP/ISP meetings and quarterly meetings.
  • Demonstrates flexibility in meeting program needs.

Qualifications And Requirements

  • Maintenance of an acceptable CORI and fingerprinting
  • Valid Driver’s license
  • Negative drug screen
  • Use of personal vehicle for work purposes
  • Commitment to the philosophy of community inclusion

EDUCATION And/or EXPERIENCE

  • Bachelors Degree in Human Service related field and/or five years combination education and/or experience in the field of ID/DD
