As a Jail-Based Care Coordinator, you will play a vital role in supporting individuals transitioning from incarceration back into the community through the Jail-Based Program. Your work will focus on care coordination and collaboration with a multidisciplinary team of community organizations. This is a highly visible, forward-facing position that serves as a key liaison with County leadership.
Key Responsibilities
Provide direct support to clients referred to the program, helping them navigate reentry into the community.
Conduct regular in-person visits to local jail to engage with clients, build rapport, and assess needs.
Facilitate weekly staffing with program partners.
Conduct intakes for all new program clients.
Collaborate with community partners, referral sources, and agencies to ensure a seamless continuum of care.
Assist clients in identifying and accessing essential services, including housing, employment, healthcare, and other resources.
Make appropriate referrals and follow up to ensure service connection and continuity.
Maintain accurate and timely documentation of client interactions, case notes, and progress.
Participate in team meetings and interagency collaborations to support client outcomes and program goals.
