Part-Time or Per Diem Physical Therapist (PT/PTA) - Home Health Visits
We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting.

Responsibilities:
Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.
Job Types: Part-time, Per-diem
Expected hours: 5 – 20 hours per week

Responsibilities:
- Provides care and services in accordance with the plan of care.
- Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
- Documents care provided in an accurate and timely manner.
- Provides teaching to clients and families.
- Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
- Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.
- Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company
- Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
- Attends continuing education programs.
- Other duties assigned by the department manager.
- Associate's degree and graduation from an accredited Physical Therapy program.
- One year of physical therapy experience preferred.
- Possesses and maintains CPR Certification
- Possess valid driver's license
- Demonstrates responsibility for self-direction
- Demonstrates the ability to learn and utilize computer technology
- Effective communication skills
- The ability to work in a team-oriented environment as well as independently
Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.
