This position is eligible for a $5,000.00 sign on bonus 1/2 payable after 90 days of employment and the remainder after a year of employment

Major Responsibilities

Coordination of POST clinic in collaboration with multidisciplinary team, both clinical and non-clinical

  • Provides coordination of patient care including health education.
  • Assists in identifying and tracking patients as they become eligible.
  • Provides initial POST clinic contact for entry to POST clinic. Explain and describe the typical POST clinic process to patient/family
  • Coordinate the multidisciplinary team for monthly clinic schedule.
  • Coordination of documentation and chart abstration. Organization of a personalized comprehensive treatment summary with recommendations for survivorship care.
  • Collaborates with physicians, management, APN and other healthcare professionals to assure the delivery of quality care and patient satisfaction.
  • Assures accurate reporting of data. Maintains POST clinic patient database
  • Ensures compliance with all regulatory agency standards for all clinic policies and procedures.
  • Promotes patient safety by reporting of issues through established channels and participating as requested in safety initiatives

Participates in development of a plan for teaching, focusing primarily on support of patient/family.

  • Recommends and helps to coordinate health education programs for patients.
  • Coordinates follow-up evaluation with providers and communicates patient evaluation to patient/family and primary care physician.
  • Patient advocate for navigating the health care system

Assures courteous, professional and efficient working relationships with our internal and external customers (Patients, medical, nursing and clerical staff).

  • Works with providers to respond to POST patient triage calls in a timely and appropriate manner.
  • Participates in developing and maintaining quality management activities.
  • Enforces appropriate policies related to clinical nursing duties and patient care.
  • Assists in retrieving patient information prior to clinic day including patient history, assessment and previous medical treatment.
  • Serves as a clinical resource to staff.
  • Tracks and responds to issues/concerns of patients and families in a timely manner. Refers complex issues to management.

Participation in professional development of self and staff through meetings.

  • Participates in monthly staff meetings.
  • Participates in and collaborates with the multidisciplinary team on CQI programs at Advocate Children’s Hospital.
  • Keeps abreast of current standards of care via literature, educational events, internal and external resources and networking.
  • Seek out educational opportunities.
  • Participate in activities to enhance professional growth by involvement in professional organizations and continuing education.
  • Maintains clinical expertise pertinent to department.

Licensure

Nurse, Registered (RN)

Education/Experience Required

  • Bachelor of Science in Nursing (BSN) and certification in your current area of specialty within 2 years or 2,000 hours.

Knowledge, Skills & Abilities Required

  • Good communication skills.
  • Good organizational skills.

Current Illinois RN license. -Obtain and maintain PALS

Physical Requirements And Working Conditions

  • Ability to be flexible with work hours.
  • Ability to communicate with patients/families and respond in a timely manner.
  • Ability to handle multiple tasks and complete work within short time frames.
  • May be exposed to bodily fluids, hazardous materials or life-threatening diseases.
  • Ability to recognize needs and behaviors of the following age groups: pediatric (newborn, toddler and school age), adolescent, adult and older adult.
  • Evidence of this can be shown by recognizing levels of development and modifying approach and exhibiting communication skills necessary to interpret age specific responses.

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
