The Behavioral Health Assistant supports the care coordination team by facilitating access to behavioral health services through direct patient engagement, scheduling and data management.
Essential Functions
- Initiate pre-screening of applications for patients referred to the BHI Program
- Schedule initial visits and follow-up appointments in coordination with care managers, coordinators, and clinicians
- Accurately relay patient information to the BHI care team
- Perform data entry in the BHI system and electronic medical record (EMR)
- Communicate with patients to ensure understanding of their appointments and care process
- Other related tasks as assigned by manager
- High school diploma required; college degree preferred
- Bilingual in English and Chinese (Cantonese and Mandarin) required
- Prior experience in a medical office or healthcare setting preferred, but not required
- Excellent communication, organization, and time management skills
- Ability to learn and navigate electronic health systems and databases (training provided)
