The LPN Clinical Coordinator supports clinical staff and the supervisor through staff assignment, scheduling, and coordination.
- Manages intake, hospital/discharge processes, and provider referrals.
- Provides administrative support, maintains scheduling, and assists with patient visit coordination.
- Handles intake calls, documents accurately, and tracks patient information.
- Participates in educational, quality, and special projects.
- Requires an active NYS LPN license, strong communication, organizational skills, and computer proficiency.
