- High school diploma or equivalent.
- Have three (3) years of responsible full time clerical experience.
- Must possess a knowledge and understanding of co-occurring disorders (mental health & substance abuse).
- Collaborate with primary care physicians, mental health specialist, and other healthcare professionals to ensure that individuals receive comprehensive integrated care for both physical and mental health.
- Monitor initial and ongoing health screenings, including but not limited to medical history, behavioral health issues, and any co-occurring disorders.
- Work with multidisciplinary teams to support the development of personalized, holistic person-centered treatment plans that address both mental health disorders (e.g., depression, anxiety, substance use) and physical conditions (e.g., diabetes, hypertension).
- Gather materials and/or resources to share with care teams.
- Monitor individuals progress and facilitating regular follow-up appointments to ensure continuity of care. This includes providing referrals and resources to specialist services and/or MyCare, when needed.
- Pulling, filing, auditing, and maintaining charts for the team assigned. This includes completing change forms and auditing and maintaining charts in accordance with Utilization Review, MCET, the Department of Community Health (DCH), OCHN, and PCHP.
- Closing Team charts.
- Co-facilitating Team meeting minutes including huddles regarding team-based care, integrated health care and other population health.
- Completing vitals and documenting in EHR system.
- Prepare and analyze metrics, provide coordination & outreach, and discuss referrals for groups & EBP's.
