About the Role:

Join Skilled Nursing Pharmacy as a Pharmacist and make a difference in the lives of patients in our community. In this dynamic role, you will collaborate with healthcare professionals to ensure safe and effective medication management for skilled nursing facilities.

Responsibilities:

• Review and verify medication orders for accuracy and appropriateness.
• Provide medication counseling and education to patients and healthcare staff.
• Monitor patient medication therapies for safety and efficacy.
• Collaborate with multidisciplinary teams to optimize patient care.
• Participate in quality assurance and medication safety initiatives.
• Stay updated on pharmacy laws and regulations to ensure compliance.
• Assist in training and mentoring pharmacy staff and interns.

Requirements:

• PharmD degree from an accredited pharmacy school.
• Active California pharmacist license in good standing.
• Minimum of 1 year experience in a pharmacy setting, preferably in long-term care.
• Strong communication and interpersonal skills for patient interaction.
• Detail-oriented with excellent organizational abilities.
• Ability to work collaboratively in a fast-paced environment.
• Proficient in pharmacy software and medication management systems.
