POSITION SUMMARY

Under the direction of the Prevention Department-SAFE Program Manager and following established procedures, receives and coordinates referrals for all SAFE and Prevention Department Programs and engages families in services. As part of the SAFE Services team, job performance contributes to achieving the goals found on the treatment plan and/or developed by the treatment team. The performance of the typical responsibilities and duties requires the ability to perform the job duties and explain to/instruct others, involving job-related matters generally limited to moderate complexity.

Typical Responsibilities And Duties

  • Receives and reviews referrals from the Call Center and internally.
  • Engage families within one business day of receiving the referrals.
  • Opens clients within 10 business days.
  • Completes client’s intake paperwork.
  • Pre-admission and admission that includes mapping clients and entering demographics.
  • Inputs and gathers pre- and post-surveys.
  • Uses a variety of culturally competent engagement strategies to engage families in services, including letters and phone calls.
  • Works with CPM to expeditiously assign the families to direct care staff.
  • Supports the collection and sharing of information and data required for treatment and outcomes using a variety of assessment, outcome, and satisfaction tools.
  • Assists with gathering required information for state and county reporting.
  • Ensures all referrals are pre-screened, with contact information.
  • Coordinate meetings as required by management, e.g., send invites, taking minutes, getting/providing essential items for meetings, managing/distributing mail, etc....
  • Manage essential documentation.
  • Consumer Perception Surveys, and data utilized for reports and outcome management.
  • Understands Wraparound Philosophy and integrates principles into their daily job performance.
  • Supports greeting clients and community members at the agency front desk.
  • Provide sound customer service and direction regarding client questions and feedback.
  • Supports with translation when needed during intake, assessments, etc.
  • Completes data entry and client screenings, including using both internal and external databases.
  • Supports children and families navigating systems, including eligibility requirements and processes.
  • Generates reports, including but not limited to financial reports, monthly school reports, school referrals, school referral log, etc.
  • Supports with check requests for flex fund use.
  • Supports the coordination of department events (baby showers, engagement parties, holiday parties, celebrations)
  • Demonstrates a high level of competence in working with children and families to assist them in attaining permanency, safety, well-being, and their self-determined quality of life.
  • Ability to engage clients in a culturally competent and dynamic manner and abide by time-sensitive deadlines.
  • Provides quality, culturally proficient, trauma-informed, and relevant ancillary services as part of a treatment team.
  • Possess knowledge of community-based resources and the ability to link and refer families to resources in the community based on their assessed needs.
  • Documents necessary information as required within the Program, promptly and consistently meets or exceeds agency standards, including Intake Forms.
  • Maintains professional standards regarding client rights.
  • Demonstrates an understanding of confidentiality policies, mandated child abuse reporting, and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question.
  • Demonstrates the ability to think creatively and intervene/respond in crises.
  • Utilizes supervision, consultation, and training to provide ongoing high-quality services to children and their families effectively and efficiently.
  • Demonstrates the ability to work independently. Actively uses supervision and consultation from his/her supervisor and other team members.
  • Every employee has the opportunity and responsibility to participate in one or more activities each year that support the performance quality improvement (PQI) of RCS programs and services. Activities might involve membership of a PQI committee or workgroup, service as a peer reviewer, data collection for PQI and evaluation purposes, completion of PQI questionnaires and surveys, or participation in focus groups, or other activities as identified by PQI committees or work groups.

MINIMUM QUALIFICATIONS

Education, Training, And Experience

  • Related BA, or AA & 2 years of related experience, or 6 years of direct service with children and families, preferred.
  • Experience working in a children’s mental health setting, emphasizing a family systems perspective, is desirable.
  • Basic skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication.
  • Bilingual/ Bicultural (Spanish/English) and/or other threshold languages preferred.

License(s)

  • California driver’s license and good DMV report (and ability to be insured by RCS’ insurance carrier).

Knowledge And Skills

  • Demonstrated ability to provide quality, culturally competent, and relevant educational and preventive services to at-risk children and families.
  • Strong sensitivity to the diverse cultural needs of children and families served.
  • Proficient oral and written communication skills.
  • Proficient organizational and time management skills.
  • Ability to work independently, exercise good judgment, making decisions & problem-solving.
  • Ability to work with confidential information required.
  • Proficiency in Microsoft Office applications.
  • Must have reliable transportation, a valid driver’s license, and minimum state-required automobile insurance.
  • Must be available evenings and/or weekends to fulfill job responsibilities.

Special Requirements

  • Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations.

Physical And Mental Requirements

The following are required in day-to-day performance of the duties of this position:

  • Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday).
  • Bending, squatting, kneeling, twisting, lifting, reaching, and pushing/pulling are required occasionally (under 1/3 of the workday).
  • Climbing and crawling are generally not required.
  • Driving/Traveling to client locations such as school, home and/or community setting.
