Description

PURPOSE:

This position will provide referrals and supportive services to children and families involved in child welfare. They will track and monitor the child’s progress, aid in the coordination of behavioral health services, and act as a family support to help stabilize youth in the least restrictive levels of care. May involve being on call (telephone) some evenings and weekends (rotating schedule).

Key Responsibilities

  • Provide behavioral health support and referrals to families involved with child welfare.
  • Monitor behavioral health needs for children and families.
  • Coordinate services and community outreach as needed.
  • Participate in staffing's intended to stabilize placement with behavioral health services.
  • Track the funding and help coordinate step down services for children in higher levels of care.
  • Work with families and providers to safely return children to their home environment with behavioral health services and family support.
  • Identify and build positive relationships with community providers.
  • Act as a resource to the CMO in helping to identify the appropriate services to keep children in the least restrictive level of care possible.
  • Field visits with families on an as needed basis.
  • Document work into statewide computer system.
  • Routinely meets with supervisor to review plans, issues, and tasks.
  • Attend staff development training for professional growth.
  • Monitor email on a daily basis. Circulate pertinent information to unit staff members as appropriate.
  • Other related duties may be assigned and/or special projects may be requested.

Pay range- $45,000.00 to $48,000.00

Requirements

EDUCATION / EXPERIENCE:

Bachelor's degree in the area of psychology, social work, or related human service field or a minimum of three years' experience working with troubled youth and their families and/or mental health services, including administrative responsibility for compliance.
