Description

Astrana is seeking an LVN to focus on closing clinical quality gaps for elderly patients over the phone. As a HEDIS LVN, you will be responsible for performing a high volume of outbound calls per day in an effort to connect with patients and ensure they are on track to prevent negative health outcomes. You will be responsible for obtaining a blood pressure reading, as well as scheduling patients for Annual Wellness Visits and other preventative health screenings (mammograms, cervical cancer screenings, colorectal screenings, etc). What You'll Do

  • Provides direct patient care as defined in the State Nurse Practice Act.
  • Through telephonic outreach, close HEDIS quality measures gaps in care for CBP, COA, OMW, and other pertinent open gaps in care under nurse practitioner supervision.
  • Completes 50-70 outbound calls per day, with a focus on closing quality gaps.
  • Implements current nursing practices following a comprehensive assessment and the plan of care.
  • Provides accurate and timely documentation of patient services to reflect the plan of care.
  • Provides patient and family/caregiver education and information pertinent to diagnosis and self-care.
  • Reports directly to the nurse practitioner for any urgent patient medical issues or conditions.
  • Once monthly, performs in-home bone density scans, provides glucometer kit teaching, and utilizes BP machines.
  • Participates in personal and professional growth and development. Also participates in quality assessment performance improvement teams and activities.
  • Performs other duties as assigned by the supervising nurse and/or nurse practitioner.


Qualifications

  • Active and unrestricted LPN/LVN lisence in CA.
  • Maintains current CPR certification.
  • Minimum of one year of experience in a hospital setting, home health, or community health.
  • Complies with accepted professional standards and scope of practice.
  • Demonstrates good verbal and written communication skills, as well as organizational skills, in a fast-paced environment.
  • Highly motivated and able to work with multiple teams simultaneously.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
  • Understands the philosophy of health care managed care concepts.
You're great for this role if you:
  • Speak Cantonese, Mandarin, or Spanish.
  • Have experience with telephonic outreach.

Environmental Job Requirements and Working Conditions

  • The is a full-time position, where you will complete 5 8-hour shifts per week. We are flexible on days per week, except we do not operate on Sunday. We offer flexible start times as well (start times b/w 8 AM - 10 AM).
  • This is a hybrid position where you will work from home 90% of the time and in patient homes 10% of the time. Home visits will take place throughout the San Gabriel Valley and Los Angeles County.
  • The target pay range for this role is $30/hr - $32/hr. The salary range represents our national target range for this role.
