Position Summary
We are seeking a proactive and detail-oriented Population Health Data Analyst to join our healthcare team. This critical role is responsible for extracting, analyzing, and interpreting data from various systems, including our clinic EHR and external data sources such as ACOs and other partner organizations, to ensure accurate reporting and optimal performance in population health initiatives. The ideal candidate will not only be technically skilled in working with data but also able to communicate effectively with healthcare providers and stakeholders to drive improvements in care delivery and compensation.

Essential Job Responsibilities
Data Integrity & Reconciliation
- Ensure alignment between internal EHR data and external sources used by ACOs and partner organizations for compensation and performance evaluation.
- Identify and resolve data discrepancies through ongoing data validation processes.
- Design, build, and distribute population health reports tailored for providers, leadership, and other stakeholders.
- Analyze data to identify trends, performance gaps, and opportunities for improvement.
- Translate complex data findings into actionable insights for clinical teams.
- Create and maintain an internal, user-friendly dashboard that tracks provider performance related to population health metrics.
- Ensure dashboards are updated regularly and reflect the most accurate and actionable data.
- Collaborate closely with clinical teams to explain data insights and support quality improvement initiatives.
- Deliver clear, concise communication about metrics and data-driven goals.
- Work proactively to identify new ways that data can be used to improve population health performance and provider compensation.
- Assist in the design of initiatives and strategies that leverage data to drive measurable improvements.
Education:
- Bachelor’s degree in Data Analytics, Public Health, Health Information Management, Statistics, or a related field (Master’s preferred).
- Experience in a healthcare analytics or population health setting.
- Strong proficiency in data tools (e.g., SQL, Excel, Tableau).
- Experience working with EHR systems and data warehouses.
- Exceptional analytical, problem-solving, and critical-thinking skills.
- Proven ability to clearly communicate complex information to clinical and non-technical audiences.
- Highly organized, self-motivated, and proactive in identifying areas for improvement.
- Understanding of healthcare quality metrics, value-based care, and population health concepts.
- Familiarity with CMS quality measures and ACO performance benchmarks.
- Experience with data governance and HIPAA-compliant data handling.
- Ability to work both independently and collaboratively in a fast-paced environment.
Performance Requirements:
- Health management experience, specifically Population Health management, a must.
- Skilled in the use of electronic medical records and clinical care processes.
- Skilled in effective education and facilitation of quality and cost efforts in a medical practice.
- Ability to create and use databases and spreadsheets.
- Ability to effectively communicate and motivate medical practitioners and all other levels of a medical practice.
- Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.
- Position is in a well-lighted office environment.
- Involves sitting approximately 90 percent of the day, walking or standing the remainder.
