- Licensed Practical Nurse (LPN/LPT/LVN) provides direct and indirect in-home care following organizational policies and physician orders.
- Documents patient progress, communicates with families and physicians, and manages medications within scope of practice.
- Performs health assessments, provides patient education, and assists clients in self-care and safety measures.
- Completes required assessments, supervises caregiver efforts, and ensures compliance with regulatory standards.
- Maintains confidentiality, uses safe body mechanics, and reports incidents as needed.
- Works closely with the nursing team to plan, implement, and evaluate care programs, attending meetings and training.
- Qualifications include an accredited nursing degree, valid state license, CPR certification, and preferred home health experience.
- Strong communication, organizational, and critical thinking skills are essential, with the ability to work independently or in a team.
- Salary is $31/hour, with flexible hours including weekends and holidays as required.
