- Seeking a Registered Nurse Case Manager with Colorado RN license or compact license, recent grads welcomed and trained.
- Must have a valid driver’s license and weekend availability with rotating on-call duties.
- Provides in-home patient care, serving as liaison among clients, their families, and community agencies.
- Responsible for admissions, ongoing assessments, care planning, medication administration, and supervision of LPNs and aides.
- Reassesses patients after hospital stays, coordinates discharge planning, and ensures regulatory compliance.
- Communicates regularly with physicians and team members, documents care, and educates patients and caregivers on self-care.
- Contributes to quality improvement initiatives and maintains high service standards.
- Position offers full-time and part-time options with a salary range of $65,000-$75,000 annually and a 401(k) benefit.
