Key Responsibilities
- Make timely welcome phone calls to newly referred patients or their caregivers
- Clearly explain our home health services, what to expect, and next steps
- Verify key information (e.g., patient availability, contact details, primary caregiver, insurance info)
- Address patient or family questions or concerns, escalating when needed
- Document all communication in the EMR or designated platform
- Coordinate with intake, scheduling, and clinical teams to ensure seamless patient onboarding
- Follow up on incomplete or pending intake information
- Maintain excellent customer service and communication standards
- Prior experience in a healthcare, home health, or patient services role preferred
- Strong communication and phone etiquette skills
- Empathy and professionalism in interacting with seniors and caregivers
- Organized, detail-oriented, and able to work independently
- Basic proficiency in using EMR systems or patient databases
- Bilingual (Spanish/English) is a plus.
