What You’ll Need
- A bachelor’s degree in human services.
- A valid driver’s license.
- A willingness to learn—we’ll provide all the training you need!
As An Aftercare Case Manager, You’ll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. You’ll
- Focus on risk management and help families maintain stability during the aftercare period.
- Provide crisis intervention and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
