InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services.
Our community Health Care Centers are located at 281 Main St. East Hartford, 40 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area.
We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Accountable for acting as part of a team in the service delivery plan for persons with mental health and or addiction disorders
and the care, guidance, supervision and training of clients in a supported or supervised housing program.
Essential Duties And Responsibilities
Qualifications:
Knowledge of modern methods of working with persons with mental health /addiction disorder and related conditions;
knowledge of safety, hygiene and health practices and procedures; knowledge of state and federal assistance programs;
knowledge of relevant state and federal laws, statutes, regulations and procedures; some knowledge of medication
administration and effects of medication; considerable interpersonal skills; oral, written, and computer communication skills;
ability to recognize and report behavioral changes; ability to coordinate and collaborate effectively with individuals, groups
and families; ability to work independently and make decisions.
Experience & Education
A Bachelor’s degree in a health care field and two (2) years of experience using case management techniques in a Behavioral
Health setting. A Master’s degree in a behavioral health discipline may be substituted for one (1) year of the General
Experience. A current Connecticut license as a registered professional nurse and two (2) years of experience working in a
psychiatric setting may be substituted for the Bachelor’s degree. On occasion, extensive community mental health
experience with seriously and persistently mentally ill persons may be substituted for the degree requirements noted above.

Essential Duties And Responsibilities
- Implements programs and monitors clients within a supported/supervised living program
- Functions as a member of an interdisciplinary team and participates in service delivery plan process for clients
- Provides guidance and support for clients to develop independent living skills
- Participates in and assists clients with activities of daily living
- Assists clients in accessing goods, services and supports required for independent living; reinforces appropriate client behavior through modification techniques
- Responsible for clients health and safety
- Observes and reports client behavior
- Provides concise and accurate reports and documentation
- May make recommendations on policies or standards
- May administer first aid in emergency situations
- Performs related duties as require
- All agency staff are required to attend all mandatory department/agency meetings and trainings*
