This role provides integrated behavioral health and substance use services within primary care settings.
- Conducts evidence-based screenings, assessments, and crisis interventions tailored to diverse patient needs.
- Develops and modifies treatment plans, evaluates progress, and facilitates appropriate referrals and discharge planning.
- Collaborates with medical providers and staff to ensure coordinated care and effective communication.
- Maintains documentation compliance, adheres to policies, and utilizes electronic health records.
- Engages in ongoing education, cultural competence, and adapts services to diverse populations.
- Requires a Master’s degree, state licensure, with preferred experience in outpatient mental health/substance use, especially in primary care, and multilingual skills are a plus.
