- Provides integrated behavioral health and substance use services within primary care, supporting prevention, intervention, and recovery.
- Conducts screenings, assessments, and crisis evaluations using evidence-based practices tailored to diverse populations.
- Develops and modifies treatment plans, evaluates progress, and facilitates appropriate referrals and discharge planning.
- Collaborates with medical providers and staff, ensuring coordinated care and effective communication.
- Maintains documentation in compliance with policies, standards, and billing procedures.
- Demonstrates cultural competence, addresses disparities, and adapts services to patient needs and norms.
- Uses electronic health records and meets continuing education requirements.
- Requires Master’s degree in Social Work, Counseling, PsyD, or PhD, Maryland licensure, and preferred experience in outpatient mental health/substance use services, with bilingual skills advantageous.
