Service Coordinator-Full Time

Objective:

Allwel is looking for a committed, energetic Service Coordinator to work with people with traumatic brain injury as well as those with other long-term disabilities, such as dementia. This position involves working with people in their homes and helping connect them with the services they need to keep them at home safely. It is highly rewarding and relationships with clients often last for years. This is a hybrid position, with a mixture of in person and remote work. The Service Coordinator supports the operations, programs and activities for services provided under the NYSDOH Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (TBI) and/or Nursing Home Transition and Diversion (NHTD) Waiver.

The Service Coordinator will:

• Facilitate the Initial Service Plan and waiver program eligibility
• Coordinate multiple services among multiple providers
• Facilitate the acquisition, oversight, and delivery of services
• Conduct monthly face-to-face visits
• Facilitate team meetings
• Complete annual service plans and other required documentation
• Address problems, crises and emergencies that may arise
• Provide advocacy as needed
• Coordinate medical appointments and transportation as needed
• Provide linkage and referrals to needed community resources
• Perform tasks/activities specific to the Service Coordination role
  
  
  

Education & Experience

The candidate shall have at a minimum, one (1) year of experience providing Service Coordination in human services and Master’s degree.

OR

Be an individual with a Bachelor’s degree and two (2) years of experience providing Service Coordination in human services.
