We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, ensuring effective discharge planning, and providing support across various settings including outpatient and inpatient environments. This role is crucial in managing the health needs of patients, particularly in geriatrics and acute care situations, while utilizing medical terminology effectively to communicate with both patients and healthcare professionals.
Responsibilities
Assess patient needs and develop comprehensive care plans tailored to individual requirements.
Coordinate discharge planning to ensure smooth transitions from hospital to home or other care facilities.
Collaborate with multidisciplinary teams to provide holistic patient service and support.
Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
Provide education to patients and families regarding health management, treatment options, and resources available.
Maintain accurate documentation of patient interactions, care plans, and outcomes in compliance with healthcare regulations.
Stay informed about best practices in emergency medicine, infectious disease care, and other relevant areas to enhance patient outcomes.
Requirements
Years of Experience: 1 year(s)
Charting System: Required, Epic
Certifications:ACLS, AWHONN Intermediate, BLS, NRP
Experience
Current Registered Nurse (RN) license required.
Experience in emergency medicine, acute care, or geriatrics is highly desirable.
Familiarity with discharge planning processes and outpatient care management.
Proficiency in medical terminology is essential for effective communication within the healthcare team.
Previous experience working in a Level I Trauma Center is a plus.
Shift: Night 3x12-Hour (18:30 - 06:00)
