Seeking a full-time Registered Nurse (RN) Hospice for ongoing staffing in Parma, Ohio.
As a Hospice RN/RN Case Manager, you will report to leadership, manage patient care, act as liaison with physicians and families, and provide compassionate support and education. You will collaborate with an interdisciplinary team to develop and implement care plans, evaluate patient needs, and maintain clinical records.
Requirements include an Ohio RN license, 1+ year of hospice, home health, ICU, or related experience, a valid driver’s license, CPR certification, and a compassionate, flexible approach. Preferred experience includes critical care, ER, or geriatric nursing.
