Yet, patients recovering at home are highly underserved. Patients can only get better at home if they receive the high-quality care that they need reliably. The existing in-home care process involves a patchwork of different vendors, all connected through faxes and phone calls. We’re building modern, robust software that solves this challenge. We currently operate across 49 states with most of the largest health systems in the US.
We’re a Series C company, backed by some of the best investors in technology and healthcare including General Catalyst, SignalFire, and Sapphire Ventures. We have ambitious expansion plans, so it’s a particularly exciting time to join the company as we’re aggressively expanding the team.
What the role consists of:
Compensation Range: $60K - $72.5K

What the role consists of:
- Contact healthcare facilities to coordinate collection of medical records
- Partner with Account Managers to resolve complex customer issues related to missing documentation, train customers’ staff to proactively avoid future documentation delinquencies
- Monitor and report care coordination metrics to department leaders
- Potentially hire, train, and manage future care coordinators
- 1-2+ years work experience in a Customer Service or healthcare Care Coordination role
- Great verbal and communication skills
- Excellent written and verbal communication skills
- High level of organization
- High attention to detail
- Great cross-team collaboration skills
- Ability to learn quickly and design brand-new processes for the company
- Be the founding member of our new Care Coordination team
- Contribute directly to a critical company-wide objective
- Fast career progression: potentially lead a team of your own within the first year of the role
- Work from our NYC office on 27th St in Chelsea (four days per week)
Compensation Range: $60K - $72.5K
