PrEP Navigator – CDPH PrEP-AP Telehealth Program

About The Position

TeleMed2U is seeking highly motivated and mission-driven PrEP Navigators to support the launch and implementation of California’s PrEP Assistance Program (PrEP-AP) in partnership with the California Department of Public Health, Office of AIDS (CDPH/OA). PrEP Navigators play a key role in guiding clients through HIV prevention services, with a focus on education, navigation, follow-up, and client retention.

This role is ideal for individuals passionate about public health, health equity, and supporting vulnerable and underserved populations through virtual care.

Key Responsibilities

  • Program Launch & General Operations Support
  • Verify pharmacy inventory for timely DoxyPEP fulfillment before provider prescription
  • Answer inbound patient calls, triage patient messages, and escalate to appropriate teams as needed
  • Triage faxes, lab results, and documentation as directed by the Program Manager or clinical team
  • Provide administrative support to ensure patients are scheduled, followed up with, and engaged in care
  • Client Navigation & Education
  • Document all outreach attempts and patient interactions in internal systems per protocol
  • Collaborate with Enrollment Workers and clinical staff to ensure continuity of care
  • Attend all CDPH/OA-required training sessions and monthly calls
  • Maintain confidentiality and comply with CDPH/OA standards, including signing and adhering to CDPH 8689 (Confidentiality Requirements)
  • Escalate any client issues, unresolved barriers, or system concerns to the Program Manager

Cross-Training & Integration Expectation

This role is part of a government-funded program and may evolve based on contract terms. Cross-training and flexibility are essential to support overall program success and potential integration with other TeleMed2U services.

Qualifications & Skills

  • 1–2 years of experience in community health, case management, or public health navigation preferred
  • Understanding or willingness to learn HIV prevention methods including PrEP, nPEP, and DoxyPEP
  • Passion for serving high-priority and underserved populations
  • Ability to communicate clearly and compassionately across diverse communities
  • Highly organized with attention to detail and documentation
  • Bilingual (English & Spanish) language skills strongly preferred
  • Comfortable working remotely and using telehealth platforms and communication tools

Physical Requirements 

  • Prolonged periods of desk work and computer usage. 
  • Ability to lift up to 15 pounds.
