- Evaluate effects and monitor and communicate progress
- Document patient care services
- Valid NY State PT License
- Master's degree from an accredited physical therapy school
- CPR Certification (Preferred)
- Good organization skills
- Good written skills
- All rates offered are based on position, years of applicable experience and employment type*** Rate ranges shown include levels for per diems with no benefits and Independent Contractors
