Responsibilities for Case Manager:

  • Experience as a Social Worker, Registered Nurse or Licensed Practical Nurse.
  • Experience as a Case Manager or Reimbursement/ Utilization Coordinator in a SNF setting required.
  • Discharge planning experience is required.
  • Ability to work collaboratively and effectively with the interdisciplinary team members, residents/patients, families, and external care team members.
  • Skilled in assessment, planning, and managing residents utilization management of care
  • Ability to organize and prioritize work and to support the resident/family/caregiver needs
  • Excellent verbal and written communication skills, expressing self in a clear, concise, and professional manner

Qualifications for Case Manager:

  • Must have a valid driver’s license since traveling is a necessity
  • RN/LPN/LSW preferred
  • Associate's degree in an appropriate field. Bachelor’s degree preferred.
  • Must be willing to work with multiple residents at once
