Position Summary:

The Registered Dietitian (RD) in a Certified Home Health Agency (CHHA) provides nutritional assessment, counseling, and support services to homebound patients. The RD works as part of an interdisciplinary healthcare team to ensure optimal nutrition care, enhance patient outcomes, and promote wellness and recovery in the home environment.

Essential Duties and Responsibilities:

  • Conduct in-home nutritional assessments of patients based on medical history, dietary intake, physical examination, and lab results.
  • Develop and implement individualized nutrition care plans aligned with physician orders and patient goals.
  • Educate patients and caregivers on therapeutic diets, nutritional guidelines, and food preparation tailored to individual medical needs (e.g., diabetes, heart disease, kidney disease, wounds, malnutrition).
  • Collaborate with speech therapists, nurses, physicians, other therapists, and other healthcare professionals to coordinate comprehensive patient care.
  • Monitor and evaluate the effectiveness of nutrition interventions and modify care plans as needed.
  • Document all patient interactions, assessments, interventions, and outcomes in the electronic health record (EHR) in a timely and compliant manner.
  • Participate in case conferences, staff meetings, and quality improvement initiatives.
  • Ensure compliance with state, federal, and CHHA regulations and standards, including Medicare Conditions of Participation.


Qualifications:

  • Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field from an accredited institution.
  • Completion of a supervised dietetic internship or coordinated program.
  • Current Registered Dietitian (RD) credential by the Commission on Dietetic Registration (CDR).
  • Licensed/Certified as required by the state in which services are provided.
  • Minimum 1 year of clinical experience, preferably in a home health or community health setting.
  • Strong knowledge of medical nutrition therapy and chronic disease management.
  • Proficiency with EHR systems and basic computer applications.
  • Excellent communication, teaching, and interpersonal skills.
  • Reliable transportation and valid driver’s license (for in-home visits).
  • Ability to work independently in patient homes with professionalism & compassion.


Preferred Qualifications:

  • Experience with Medicare-certified home health services.
  • Bilingual skills a plus.
  • Certification in diabetes education, renal nutrition, or wound care preferred.
