Summary
Leads, manages, and facilitates transformational community and systems change efforts, by leveraging data and community input, to improve the health of vulnerable populations within the Leonard Parker Pool Insitute for Health (LPPIH) team. Develops and implements innovative and collaborative strategies to address social determinants of health and vital conditions in the community. Supports internal stakeholders and collaborative external partners to understand the root causes and dynamics of social drivers that impact health in order to design and iterate strategies to address those social drivers and vital conditions within a place-based framework. Develops and manages relationships with local stakeholders across all levels of the community, including residents, community-based organizations, systems leaders, and business leaders. Develops and manages relationships with state and national partners as well as cross-sector health improvement and community development initiatives.
Job Duties
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Job Duties
- Manages and oversees LPPIH’s comprehensive strategies focusing on vital conditions within our neighborhood development, place-based approach to health improvement.
- Develops and maintains meaningful, collaborative relationships with community partners, systems leaders, and community members in order to successfully manage and nurture the ecosystem of partners within the neighborhood development work.
- Implements, manages, and oversees cross-sector leadership development and capacity-building efforts through initiatives like the Pool Fellowship for Health and Pool Speaker Series.
- Convenes and engages diverse stakeholders and partners to develop strategies that influence health and advance community innovations that improve health.
- Oversees team members that are responsible for the development of community innovation strategic efforts to improve health equity. Works with communications and evaluation colleagues to measures and communicate progress towards goals and primary learnings.
- Supports and collaborates with health system colleagues throughout the healthcare network, ensuring LPPIH’s community work connects to and supports the overall mission of the healthcare system.
- Manages relationships with state and national partners and field experts related to population health, community development, and public health (e.g., CDC, StriveTogether, BUILD Health).
- Translates best-practices from community development and related fields to our local community and manages complex, multi-faceted collaborations, coalitions, and ecosystems.
- Documents protocols, progress reports, and other summaries and reports to ensure our work is captured and communicated to a variety of audiences.
- Master’s Degree Social Work, Public Health, Community Health, Human Development, or Public Policy, Business
- 5 years Experience of managing community-based initiatives and relationship development with community-based organizations and other large sectors and systems and
- 5 years Experience in research and documented record of peer-reviewed publications and presentations at national conferences. and
- 5 years Experience in community health and/or health delivery system translation improvement work.
- Ability to manage various levels of personnel and mentor personnel.
- Demonstrated knowledge of public health and community development best-practices.
- Demonstrated knowledge of population health management in order to evaluate and scale innovation pilots.
- Demonstrated ability to communicate effectively with a wide range of clinicians, scientists, and stakeholders from diverse backgrounds.
- Excellent written and oral communication skills.
- Ability to develop and maintain relationships with external stakeholders and community-based organizations.
