We are seeking an Intake Manager responsible for oversight of the intake department and full cycle intake process. The intake department processes patient referrals from a variety of referral sources, including, but not limited to hospitals, doctor’s offices, rehabs, assisted living facilities. The Intake Manager will work directly with branches and leadership to timely and effectively coordinate patient care and referral response times. The Intake Manager oversees a department of 3-4 Intake Coordinators.
The Manager of Intake Management will provide oversight of intake coordination, tracking and timely follow-up for all referrals made to the agency ensuring compliance with Aveanna Healthcare policies, procedures and federal/state regulations and guidelines. The position will provide an end to end 5 Star referral experience differentiating Aveanna Healthcare in their marketplace and upholding the Aveanna Healthcare Mission in every interaction.
R Esponsibilities And Job Function
Manages team and process to ensure as the first line of communication to physicians, patients, families and referral sources there is effective coordination of home care services referrals. Monitors team output to for proactive and professional communication to all parties and timely completion of required documentation, system entry and transfer of pertinent information to client ’ s physician, therapists and agency staff members. This position must proactively lead a high performing team to drive growth and deliver exceptional customer experience for each referral. Will collaborate within and across HHH leadership teams.

- This position may have weekend responsibilities depending on the needs of the business.
- Ensures the intake process follows all applicable local, state or federal regulatory requirements and Aveanna Healthcare policies and procedures.
- Ensures high quality service levels for referral sources, providers, physicians and staff monitoring their ongoing feedback for best practice experience.
- Maintains open lines of communication with Leadership to ensure issues, challenges and barriers are addressed timely throughout the referral window.
- Coordinates with Account Executives as a mediator and liaison for customers, to ensure the best possible experience is obtained while engaging with our company.
- Provides team members with the appropriate training , tools, direction and motivation that enable their success.
- Acts as a backup intake coordinator as needed.
- High school diploma/GED or a college degree
- College degree preferred – Experience will be considered in placement of degree.
- Must be able to work after hours on-call schedule
- Proficient Microsoft Office skills
